[MUSIC] Hello. I'm Dr. Mikael Snitker-Magin here to talk with you about the test transmittal form used by the Educational Counseling and Disabilities Services office. The test transmittal form is a form used by faculty to convey your exam to our office. The purpose of the form is to give us information about the test conditions under which you want the student to take the exam. We need to know from you, for example, are you going to allow the student to take the exam on the same day or on a different day. We also need to know if you are going to allow the student to use additional materials that you've allowed other students in the class to use. For example, will you allow the student to use a calculator or notes? We also want to know how you want to receive the exam returned to your office. Some faculties prefer that we return the exam via e-mail, fax, or the courier. That is the third section on the form that we need to know from you. Our obligation is to administer the exam and then support you as the faculty member with any questions that you might have about exam accommodations.
